East Riding Voluntary Action Services (ERVAS) Ltd has recently been appointed as lead partner of an exciting service alongside Humber and Wolds Rural Community Council and the SHoRes in Withernsea financed by City Health Care Partnership CIC. The service aims to create a streamlined system for health and social care professionals to access voluntary and community support for individuals with an East Riding Clinical Commissioning Group GP.
The focus of the service will be on early identification of need and prevention of reliance on more specialist services. Interconnection with local community services and providers will be a fundamental part of the role along with monitoring and recording service activity and outcomes for individuals and families.
1.1 The service model will contribute to:
1.1.1 Enhancing quality of life for people with care and support needs;
1.1.2 The integration agenda by reducing unnecessary GP appointments;
1.1.3 Releasing social and health care capacity to focus on those individuals needing an appropriate intervention; and
1.1.4 Improving health related quality of life for people aged 18+ (with an East Riding of Yorkshire CCG GP)
1.2 The following objectives apply to this Service:
1.2.1 To address non-clinical demands on primary care by delivering a tested intervention which connects people to sources of support within their community;
1.2.2 To provide community support to individuals who require a social prescribing intervention from a Community Link Worker (CLW), by connecting them to services, community groups, clubs, befriending schemes etc;
1.2.3 People receiving services from the CLW Service will feel supported, promoting independence and reducing isolation and loneliness;
1.2.4 The CLW Service will increase the use of local community based groups and support and identify current gaps in provision demonstrated through recorded feedback from organisations;
1.2.5 To make effective use of voluntary and community sector assets by improving connections and relationships between local communities and voluntary sector organisations and traditional providers of health and social care; and
To improve the health and well-being of local people by connecting them to community based activities which support their independence and reduce reliance on acute or specialist services.
